A checklist of senior home care needs is an excellent place to start for identifying and assessing needs. Reviewing a compre- hensive list virtually ensures that all needs are identified and then subsequently ranked in terms of priority. In addition to fully identifying specific se- nior home care needs understanding trends in senior home care is a critical factor for consideration in the planning and selection process. Specialized Medical Home Care Providers offer very focused expert care. Such services are usually related to acute situations e.g. stroke cancer that re- quire post-acute care. The starting point in developing the plan is a thorough needs assessment iden- tifying all possible senior home care needs weighting the importance of each need and ranking the needs. Next establish clear ob- jectives take the prioritized list of needs and state an objectives for each need. Once clear objectives are established form the strategy the how to. The how is all about action steps and who spouse family member relative friend etc. is re- sponsible for each action element.
